Required Documentation to Be Eligible for Veterans’ Preference

Applicants who wish to claim Veterans’ Preference are required to submit copies of certain documents.  Click on the statement below to view the required documentation.

*Only one of the documents listed is required to be submitted

  • DD Form 214 (Member 4 or Service 2 copy) listing an Honorable or General (Under Honorable Conditions) Discharge
  • DD Form 256 (Discharge Certificate, Honorable)

*Only one of the documents listed is required to be submitted

  • Current military ID card (both sides)
  • Letter from unit commander verifying service member is in good standing 
  • DD Form 214 (Member 4 or Service 2 copy) listing an Honorable or General (Under Honorable Conditions) Discharge
  • DD Form 256 (Discharge Certificate, Honorable)

*Only one of the documents listed is required to be submitted

  • Current military ID card (both sides)
  • Letter from unit commander verifying that service member is in good standing
  • DD Form 214(Member 4 or Service 2 copy) listing an Honorable or General (Under Honorable Conditions) Discharge upon demobilization
  • DD Form 256 (Discharge Certificate, Honorable)                            

*Only one of the documents listed is required to be submitted

  • NGB Form 22 (National Guard) listing an Honorable or General (Under Honorable Conditions) Discharge
  • NGB Form 55 (National Guard - Certificate of Honorable Discharge)
  • NGB Form 56 (National Guard - Certificate of General Discharge)
  • DD Form 214 (Member 4 or Service 2 copy) listing an Honorable or General Discharge
  • DD Form 256 (Discharge Certificate, Honorable)

*Submit one of the following documents with applicant’s SSN included:

  • Front page of the most recent federal tax return listing both spouses names
  • Marriage license or certificate (if marriage occurred between joint tax filing)

AND

*In addition to submitting the verification above, a spouse of a veteran must submit one of the following documents with applicant’s SSN included

  • DD Form 214 (Member 4 or Service 2 copy) listing an Honorable or General (Under Honorable Conditions) Discharge
  • DD Form 256 (Discharge Certificate, Honorable)
  • NGB Form 22 (National Guard) listing an Honorable or General Discharge
  • NGB Form 55 (National Guard - Certificate of Honorable Discharge)
  • NGB Form 56 (National Guard - Certificate of General Discharge)

*Submit one of the following documents with applicant’s SSN included:

  • Front page of the most recent federal tax return listing both spouses names
  • Marriage license or certificate (if marriage occurred between joint tax filing)

AND

*In addition to submitting the verification above, a spouse of a current service member must submit one of the documents below with applicant's SSN included.

  • Current military ID card (both sides)
  • Letter from unit commander verifying service member is in good standing
  • Notarized statement that spouse has not remarried
  • Veteran’s Death Certificate or DD Form 1300 (Report of Casualty)

*In addition to submitting the verifications listed above, a unmarried spouse of a deceased veteran must also submit one of one of the documents below:

  • DD Form 214 (Member 4 or Service 2 copy) listing an Honorable or General (Under Honorable Conditions) Discharge
  • DD Form 256 (Discharge Certificate, Honorable)
  • NGB Form 22 (National Guard) listing an Honorable or General (Under Honorable Conditions) Discharge
  • NGB Form 55 (National Guard - Certificate of Honorable Discharge)
  • NGB Form 56 (National Guard - Certificate of General Discharge)
  • Verification from the U.S. Department of Veterans Affairs that veteran has been declared unemployable due to a service connected disability
  • The front page of the disabled veteran’s most recent federal tax return listing parent as a dependent.

*In addition to submitting the verification listed above, a dependent parent of a disabled veteran must also submit one of the documents below:

  • DD Form 214 (Member 4 or Service 2 copy) listing an Honorable or General (Under Honorable Conditions) Discharge
  • DD Form 256 (Discharge Certificate, Honorable)
  • NGB Form 22 (National Guard) listing an Honorable or General (Under Honorable Conditions) Discharge
  • NGB Form 55 (National Guard - Certificate of Honorable Discharge)
  • NGB Form 56 (National Guard - Certificate of General Discharge)
  • Proof of veteran's death while on active duty or active duty for training (DD Form 1300- Report of Casualty)
  • The front page of the deceased veteran’s most recent federal tax return listing parent as a dependent.

*In addition to submitting the verification listed above, a parent of a deceased veteran must also submit one of the documents below:

  • DD Form 214 (Member 4 or Service 2 copy) listing an Honorable or General (Under Honorable Conditions) Discharge
  • DD Form 256 (Discharge Certificate, Honorable)
  • NGB Form 22 (National Guard) listing an Honorable or General (Under Honorable Conditions) Discharge
  • NGB Form 55 (National Guard - Certificate of Honorable Discharge)
  • NGB Form 56 (National Guard - Certificate of General Discharge)
